errors in rendering and saving

I can fill this form with PDF reader, but in PDF Editor in section 12, 13 and 14 dont appear in PDFReader.

And some box disappear.

Please provide us the original file. But the better way to modify a form file, is to use Foxit Form Designer. Foxit Editor currently may not work well with Form.

ok, here is original file.

I add texts and save it with pdfeditor. after that when open in pdfreader, the text disappear.

and another errors with Foxit PDF Reader, when I edit in section B in Job title, this text is crossed link with From (mm/yy)...
or in Hours per week also cross-link
or section 14 in NAME and Location of school also

and please remove the first file I uploaded, because it have some privacy information.
