Unable to fill in PDF form

I am unable to key in any data into the attached PDF file. I can when using almost any version of Adobe Acrobat Reader though. This appears to be a Foxit bug.
Can you assist?

I can key in almost all the data, but there are some fields that won't key in. Please try the latest Reader- 2.2.

I am already using 2.2. I can not key in the SSN, Name or Zip Code. The drop-down boxes do not work properly. An item can be selected, but the value is not automatically put in the Coverage Code field.
If you use Acrobat Reader (most versions), you will see how it works.
This appears to be a bug with Foxit Reader. I have tried both v2.1 and 2.2. Neither one work.

Not sure if this is the same bug but I have a form that will let me fill it in completely...but then the next time I open it (or the next time after that or 2 times after that...it is random) it won't let anything be filled in. Very frustrating.

The only fix is to uninstall foxit reader 2.2 and then reinstall it... then it will work...at least for a while.
