Creating Bookmarks

I've looked and most likely missed it, but have a question on Bookmarks. I've successfully created my book in Word, converted the book successfully to PDF with photos, page number and all looks goo. How in the world to I create the Bookmarks say by chapter or page number in the left hand column like most PDF's?
