I have a PDF form (made in LC ES2) with extended features enabled because we want the forms to be saved.

Foxit Reader or PhantomPDF supports to fill-in form and save it.

Are you asking how to preserve the inherent resolution of images within a PDF document which you create with PhantomPDF?
