Bug can't print doc with annotations

I am currently unable to print my fillable, calculating tax form. I am able to enter in my numbers and the form does the calculations. When I try to print "Document and Annotations", I get only the annotations (also what shows up in the preview). I can print "Document" and get the document; I can print "Annotations" and get the annotations.



I am trying to use the Missouri state tax form:

I tried updating to Foxit v. 5.1.4.0104. The "Print" button at the top left of the form also does not work. Options come up, but when I select what to print, nothing happens.

I'd appreciate if someone can help me not to have to fill out my tax form by hand!

I am afraid it has nothing to do with pdf reader, but it is the problem of the file itself.
It also works same with Adobe Reader.
