Bug Active form not sending

I am trying to have an active pdf form send filled values to a server side script but I do not seem to be getting any values send.I am using the latest Foxit Reader but is giving me a feedback of "Your form submission is successful!".

I would like to have the info filled in the form to be send to a php script.
